INSTRUCTIONS FOR COMPLETING THE ON-LINE APPLICATION


General User Instructions

Click on “Apply Online” button it will open option "Candidate Application Form". For Candidate Registration process you have to fill fundamental information's, particulars and details asked for. Fields with " * " are mandatory and essential to be filled in by the candidate. Every field has clear instructions for filling up, which should be carefully read and strictly followed by the candidates while filling up the form.

Part-1 Registration

  • The candidate should first register by clicking “Apply Online”. An email confirming the registration will be sent by allotted Registration no. and Password. The Registration Number and the Password are required for subsequent operations. Hence the candidate should remember them or keep them in safely.
  • Candidate can start filling up the on-line application by logging in through " View/Edit Application".
  • The candidate should enter all required information correctly in all fields of the Apply on-line application.
  • After filling the fields, the candidate can save the information in between by using the SUBMIT button. The candidate can edit data in any field till the final submission and printout is taken. The last date for Apply on-line submission of application is March 10th,2016.

Candidate Basic Registration form:

Here the system asks for Candidate’s Scanned Copy of Personal Information including 'Type of Courses', 'Candidate Name', 'Father’s/Guardian’s name', 'Occupation's', 'Income', 'Category', 'DOB', Gender, 'Contact no.', Email-Id, Qualification details, and other relevant information’s (All Document must be Self Attested).

Photograph

  • Here the candidate is required to upload the images of recent Photograph. Images to be uploaded should be only in *jpeg format of size not exceeding 50 KB.

Demand Draft

Advised to keep your demand draft ready before taking the final printout of the application form. Also write your name , Registration Number and your address at the back side of the demand draft before dispatching the Hard Copy to Us.

Detailed Application Form:

  • On the top of the page there is a 'Declaration' for the candidates. Candidates are advised to go through the contents of the Declaration carefully. Candidate has the option to either agree or disagree with the contents of Declaration by clicking on either ‘I Agree’ or ‘I do not agree’ buttons. In case the candidate opts to disagree, the application will be dropped and the procedure will be terminated. Accepting to agree only will submit the candidate’s Online Application.
  • Personnel Details
    This section shows information about candidate personnel details i.e. Candidate name, Father’s/Guardian’s name, Mother Name, Occupation, Gender, DOB, Category, Valid Email-id and Valid Contact number.
  • Education Details
    It shows your education details. This section shows information about Exam Passed, Board/University, Year of Passing and Percentage (%) of Marks Obtained.
  • Candidate address details
    Here you will see your complete communication / mailing address (Present / Permanent) with Pin Code.
  • Declaration segment
    At the bottom of the page there is a ‘Declaration’ for the candidates. Candidates are advised to go through the contents of the Declaration carefully.
  • Declaration:-
    We here by declare that the information furnished in the application is true and correct to the best of my knowledge and belief. The original certificates will be produced for verification at the time of admission. In the event of any information furnished by us is found to be incorrect or false, I agree to rejection/termination of the candidature/ admission and forgo any claim what so ever.

APPLICATION FEE

A demand draft for Rs 2500/- for all Category (such as General / OBC / SC / ST / PD Category candidates), drawn in favour of 'Hope Ray Health Care' payable at "New Delhi". Candidate should write their Registration number, name, address and courses applied on back side of the Demand Draft. Candidate should note that the fee paid other than Demand Draft i.e., by IPO, cheques, etc. are not acceptable. Application fee once paid cannot be refunded. After submission of application form, the demand draft has to be sent to the corporate office of Hope Ray Health Care.
